Welcoming a new addition to the family is a joyous occasion, but for many mothers, it also brings physical challenges. In this article, we'll explore these challenges with experts Daryl and Lilian from Ready Fit Physiotherapy. They'll share their knowledge with regards to postpartum recovery, as well as what their recovery program entails. Let's go!

Hi Daryl and Lilian! Thank you for sharing your expertise with us. We understand that your team is experienced in helping ladies recover in their postpartum phase. Could you share with us what is your team's relevant experience in this aspect?

At Ready Fit Physiotherapy, we have a Post Pregnancy Care Programme that harnesses a perfect harmony of body analysis, recovery therapy, physiotherapy strengthening exercises, and advanced technology to treat and manage Diastasis Recti (DR), lower back pain, urinary incontinence, and loose skin.

It is always a joy to welcome new member(s) of the family. However, after giving birth, many mothers are troubled by the physical changes of their bodies. For some, they feel as if their health has been robbed. What are some of the common problems faced by mothers after delivery?

Common issues faced by postpartum mummies include Diastasis Recti, lower back pain, urinary incontinence, and oftentimes also have some loose skin. Diastasis Recti refers to a partial or complete separation of the abdominal muscles. This separation that occurs during pregnancy will put additional strain on the back muscles, leading to lower back pain in many pregnant women. Urinary incontinence occurs as a result of pelvic floor weakness. Internal pressures such as coughing and sneezing, or external pressures such as high-impact activities e.g. running and jumping, may trigger urinary leakage.

Could these postpartum issues have been prevented with earlier intervention or specific preventive measures during pregnancy, or even earlier on?

Yes, ladies/mothers-to-be are encouraged to train their body to possess strong core and pelvic floor muscles as a preventive measure before and during pregnancy. Having a strong core and pelvic floor muscles help support the spine and pelvic floor as the foetus’ weight increases through the pregnancy journey.

We hear a lot about core muscles but many of us don’t have a clear picture of what it entails. Can you tell us about the role of our core muscles in pregnancy?

The core (i.e. transverse abdominis), is the deepest layer of abdominal muscles. It forms a corset, helping to contain your internal organs. Keeping this muscle toned helps prevent other problems such as backaches and poor posture. It supports the rectus abdominis, lowering the risk of this muscle ‘splitting’, otherwise known as Diastasis Recti.

Majority of multiple mums would definitely have found it difficult to balance the above points you mentioned due to the complexity and challenges of a multiple pregnancy. How can physiotherapy help them in their recovery now?

Our program focuses on building a tailored treatment plan according to the client’s needs. The initial assessment includes Body Mass Index (BMI) calculation, skinfold measurements, waist circumference, and a thorough Diastasis Recti assessment using ultrasound.

As for treatment, we use a physiotherapy-focused approach for core and pelvic floor activation training, using a biofeedback device with mat and gym exercises, and a pilates reformer.We use advanced technology such as our High-Intensity Electromagnetic Therapy (HIEMT) machine for abdominal and pelvic floor strengthening. The electromagnetic field is effective in stimulating contractions for accelerated muscle growth and fat burns. Our WINBACK Tecar Therapy uses high radio frequency treatment that boosts healing naturally. It stimulates your body’s natural repair mechanisms, tightening up loose skin, reducing tummy bulge, and lower back pain.

For those who have missed the boat in the earlier postpartum period, would the above still be helpful for them? 

It’s never too late to start. With that being said, the earlier the strengthening process begins, the more effective the treatment would be. The recovery rate may be slower for mothers who start later but they should still see improvements.

What are some common myths or mistakes that women may encounter when it comes to postpartum recovery, and how can they be addressed or avoided?

In terms of exercising, some believe they are to avoid doing crunches or planks to prevent worsening the DR. It is important to take it slow, avoid strenuous activities, and to exercise under the guidance of professionals.

Generally speaking, how long would it take before we can see an improvement upon starting treatment? What are other factors that may affect the rate of recovery?

It usually takes around 4-5 sessions to see an improvement. There are, of course, many factors - your level of commitment, your diet, your body’s rate of recovery, and even your baseline/pre-pregnancy level of fitness. Other factors such as age, history of pregnancy (e.g. number of pregnancies, type of delivery), and starting point of postpartum strengthening (i.e. starting earlier would be more effective) may also affect one’s recovery.
